Most requests through this website collect scheduling and pre-registration details. The new patient intake form additionally gathers the health information you choose to provide, so the practice can prepare your chart before your first visit. That information is transmitted securely to the practice and is not kept on this website beyond the short time needed to deliver it (described under How Long We Keep It below). Your full, ongoing medical record is maintained through the practice's secure EHR patient portal, which operates under a separate HIPAA-compliant infrastructure.

While we hold a submission, it is stored securely in Cloudflare's infrastructure and is accessible only to authorized practice staff through a protected admin panel. How long we keep it is described in the next section.
We keep what you submit only as long as we need it. New patient intake information is delivered to the practice and then automatically and permanently removed from this website's systems within days of that delivery being confirmed, leaving only a plain record that a form was received and handed off. From that point your information lives solely in the practice's secure EHR, not on this website.
If a delivery cannot be confirmed, your submission is held rather than silently discarded, so the practice can still reach you, and it is removed once the matter is resolved.
This website does not use tracking cookies, third-party analytics, or advertising pixels. We do not build profiles from your browsing behavior.
Standard server logs may capture your IP address and browser type for security and performance purposes only. These logs are not linked to your personal identity and are not retained beyond standard security retention periods.
