This website collects scheduling and pre-registration information only. No protected health information (PHI), medical records, diagnoses, or clinical history is transmitted or stored through this system. Your full medical record is maintained through the practice's secure EHR patient portal, which operates under a separate HIPAA-compliant infrastructure.

How We Use It

Information submitted through this website is used solely to:

  • Contact you to confirm or schedule your appointment
  • Set up your patient record before your first visit
  • Verify your insurance coverage
  • Route your prescriptions to your preferred pharmacy

We do not use your information for marketing purposes, and we do not sell or rent your information to any third party.

Who We Share It With

We do not sell, trade, or transfer your personal information to outside parties. Information may be shared only in the following limited circumstances:

  • Practice staff only — authorized front office and clinical staff access submissions to prepare for your visit

  • With your consent — we will not share your information with any other party without your explicit written authorization

Submissions are stored securely in Cloudflare's infrastructure and are accessible only to authorized practice staff through a protected admin panel.

Cookies & Website Data

This website does not use tracking cookies, third-party analytics, or advertising pixels. We do not build profiles from your browsing behavior.

Standard server logs may capture your IP address and browser type for security and performance purposes only. These logs are not linked to your personal identity and are not retained beyond standard security retention periods.

Your Rights

You have the right to:

  • Request a copy of the information you submitted through this site
  • Request correction of inaccurate information
  • Request deletion of your pre-registration data prior to your first visit
